This form is for students whose high school class graduated seven or more years prior to applying for admission to Utah Tech University, formerly DSU.
The purpose is to gather High School graduation or GED completion information.

** Completion of this form will satisfy your high school transcript requirement. Therefore, if you complete this form, you do not need to provide your high school transcript for admissions.
